  • ​Keep our office informed ​of any changes that can impact your insurance coverage. This includes changes in:
    • Name, address or telephone number
    • Insurance provider
    • Number of family members
    • Medical condition(s)
  • For prescription refills, call (317) 859-3785 and choose option “1” during business hours. Your prescription will be called into your pharmacy within two business days.
  • If you are calling to ask about test results and it has been one to two weeks since your test, call (317) 781-7188 and choose option "4." Our physicians or clinical staff will return your call.
  • Co-payments are expected at the time of service. If you have questions regarding your statement, please call our billing office at (855) 376-0014 or (888) 777-7148. Our billing office can assist in arranging a payment plan, if necessary.
In cases of sudden illness:
  • If you experience a medical emergency, call 911 or go to the nearest hospital emergency room and ask the staff to contact your physician.
  • In cases of sudden illness, call our medical office. We will fit you into our daily schedule as soon as possible. It may be necessary for a physician other than your own to examine you.
  • To reach the physician on call after office hours, call (317) 202-7299.
