Patient information:
  • Keep our office informed of any changes that can impact your insurance coverage. This includes changes in:
    • Name, address or telephone number
    • Insurance provider
    • Number of family members
    • Medical condition(s)
  • For prescription refills, call (317) 862-2277 during business hours. Your prescription will be called into your pharmacy within the next 24 hours.
  • Co-payments are expected at the time of service. Payment is expected within 30 days after receiving your statement. If you have questions regarding your statement, please call our billing office at (855) 376-0014. Our billing office can assist in arranging a payment plan, if necessary.
In cases of sudden illness:
  • If you experience a medical emergency, call 911 or go to the nearest hospital emergency room and ask the staff to contact your physician.
  • In cases of sudden illness, call our medical office. We will fit you into our daily schedule as soon as possible. It may be necessary for a physician other than your own to examine you.
  • To reach the physician on call after office hours, call (317) 631-3466.
