Medical records are maintained for a period of time specified by state and federal regulations. A patient or legal guardian of a patient may request copies of his/her medical records. Proper authorization for the release of medical records is required.

This process is in compliance with Federal HIPAA guidelines. Please allow up to 15 business days for your records to be processed and mailed to the specified address.
Franciscan St. Francis Health contracts with Smart Document Solutions, LLC, for this service. There is a fee for processing your request. You will receive an invoice along with the records.
Medical records are destroyed after 10 years.
The HIM department processes release of information from 8 a.m. to 4:30 p.m. Monday through Friday.
Who is authorized to sign for release of my medical records?
- You, the patient
- Your Healthcare Representative or Healthcare Power of Attorney if you are unable to sign. Legal documents must be provided.
- If you are under 18, your parent/parents may sign for you.
- Legal guardian if appropriate. Documentation must be provided.
- For deceased patients, the Executor of the Estate is the first person who can request copies of records. Death Certificate and Estate documents must be presented.
Obtaining Birth/Death Certificates
Birth/Death Certificates for our Indianapolis hospital are available from the state of Indiana.
