Franciscan St. Elizabeth Health Diabetes Center will move from its current location at the Medical Arts Building, 415 N. 26th St. in Lafayette on the campus of the former Home Hospital, to its new location in the lower level of the Lafayette Heart Institute, 1116 N. 16th St. in Lafayette on the campus of Franciscan St. Elizabeth Health – Lafayette Central.
The Diabetes Center will close at 5 p.m. on Friday, June 29 and will reopen at 9 a.m. on Monday, July 9 as Diabetes Education in the Franciscan Healthy Living Center.
As part of the move, the Diabetes Center will receive a new phone number – (765) 428-5850 – and a new fax number – (765) 428-5851.
The Diabetes Center offers an American Diabetes Association recognized diabetes self-management education program, including blood glucose monitoring, healthy meal planning, taking medications, reducing risks and gestational diabetes management.
